Primary Care Physician Opening in Danbury, CT
* Location: Danbury, Connecticut — serving post-acute care communities
* Role: Full-time, fee-for-service (FFS) Physician
* Patient Load: 15–20 patients per day

* Estimated Annual Compensation: $255,608 – $368,076 (uncapped productivity bonus potential)

### Responsibilities
* Deliver high-quality, safe, cost-effective care in post-acute settings
* Collaborate with interdisciplinary teams and adhere to CMS guidelines
* Coordinate transitions with hospice, palliative care, and home health providers
* Complete timely documentation and billing via Gehrimed EMR, in line with Medicare standards
### Qualifications
* Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine
* Licensed to practice in Connecticut
* Willing to train experienced candidates
