We are seeking a dedicated and community-oriented Family Care Provider to join a rural health clinic team in Aledo, IL. The role includes clinic, hospital, and long-term care coverage with the opportunity to provide comprehensive healthcare for patients of all ages. Job Responsibilities

• Provide primary care services including preventive care, chronic disease management, and acute illness treatment.
• Cover clinic, hospital inpatient services, and long-term care facility patients.
• Participate in a 1:6 call rotation, including weekends as scheduled.
• Collaborate with advanced practice providers and other medical professionals to ensure coordinated patient care.
• Maintain accurate, timely medical documentation and comply with all clinical guidelines.

Qualifications

• Board Certified or Board Eligible in Family Medicine or Internal Medicine (BE within 3 years of residency) – Required
• Active Illinois medical license or eligibility to obtain – Required
• Clean malpractice history preferred; no active cases requiring supervision – Required
• Interest in practicing in a rural healthcare setting – Required
• Not accepting locum or visa candidates

Working Hours

• Monday–Friday, 8:00 AM–5:00 PM
• On-call rotation 1:6, including weekends

Skills

• Strong clinical and diagnostic abilities in primary care
• Ability to manage patients across the lifespan in outpatient, inpatient, and long-term care settings
• Excellent communication and interpersonal skills
• Commitment to rural community healthcare and patient-centered service
• Collaborative approach to working within a multidisciplinary team
