Key Responsibilities:

• Provide primary care services to short-term and long-term patients
• Diagnose and manage acute and chronic medical conditions
• Order and interpret labs, prescribe medications, and monitor treatment plans
• Coordinate care with other healthcare professionals
• Educate patients and caregivers on chronic disease management and healthy lifestyle habits
• Foster strong relationships with patients, families, and facility staff

Compensation & Benefits:

• Estimated annual compensation: $210,529 – $271,529 (no cap on productivity-based income)

Candidate Requirements:

• Board-certified or board-eligible physician
• Minimum 1 year of primary care experience required
• Post-acute care experience preferred but not required
• Active and unrestricted Pennsylvania medical license
• DEA license with prescriptive authority
