Join our Senior Primary Care Office in Deerfield Beach, Florida.
Conviva Senior Care Centers, a subsidiary of Humana Inc., is seeking a Family, Internal, or Geriatric Medicine Physician to join our growing team in this established office. Grow with us as we expand our reach to help more Seniors live better, healthier lives.
You have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinates of health are impacting health outcomes. - Guaranteed base salary + 20% bi-annual quality incentive, for total compensation up to $300,000+

Job Responsibilities
-Monday-Friday 8:00am-5:00pm; no weekends
-See 12-15 patients per day. Opportunity to spend more time with patients
-Longer authorized appointments (20-45 minutes)
-Acts as a lead and active participant and key source of medical expertise with the Care Team through daily huddles
-Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care
Qualifications
-Doctor of Medicine or Osteopathic Medicine
-Board Certified/Board Eligible in Family, Internal, or Geriatric Medicine
-Valid, unrestricted license in the state of desired employment (or willing to obtain)
-Current, unrestricted DEA license
Preferred Qualifications:
-Two to five years directly applicable experience preferred
-Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
-Knowledge of Medicare guidelines and coverage.
-Knowledge of HEDIS quality indicators.
Working Hours
Outpatient only, seeing a low patient volume under a Value-Based Care Model.
