Position Highlights:
* Physician-led, team-based model focused on coordinated, patient-centered care
* Opportunity to provide care across the continuum in a value-based care setting
* Procedures may include, but are not limited to: simple skin surgery, fracture care, joint injections, maternity care, IUD placement
* Full-time schedule options: 4 or 4.5 workdays per week
* Open to board certified or board eligible Internal Medicine physicians
* Must be comfortable working alongside and supervising advanced practice providers

Qualifications:
* Unrestricted medical license in Indiana or ability to obtain
* Board certified or board eligible in Internal Medicine or Family Medicine
* Active and unrestricted DEA license or ability to obtain before start
