Location: San Bernardino, California
Compensation: $315,000–$380,000 base + bonus + equity
Schedule: Full-time | Monday–Friday | No weekend clinic

As the Clinical Team Lead, you’ll guide a small care team while managing a manageable panel of patients through relationship-driven, longitudinal care.
Key Responsibilities

Lead and support a cross-functional care team delivering comprehensive primary care

Provide both clinic-based and occasional in-home visits to enrolled patients

Emphasize continuity and prevention rather than episodic or volume-driven care

Collaborate closely with behavioral health, social work, therapy, and nursing professionals

Participate in a light call rotation (phone-only, a few nights or weekends per month)
Compensation and Benefits

Base salary: $315,000 to $380,000, depending on experience

Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
