Job Responsibilities
• Diagnose and manage acute and chronic medical conditions in an outpatient setting.
• Perform routine physical exams and preventative screenings
• Develop individualized treatment plans based on patient needs and evidence-based practices.
• Coordinate care with specialists, hospitals, and ancillary providers to ensure continuity of care.
• Maintain accurate, timely, and detailed electronic medical records (EMR).
• Educate patients on wellness, disease prevention, and healthy lifestyle choices.
• Participate in quality improvement initiatives and performance metrics.
• Collaborate with a multidisciplinary care team, including nurses, social workers, and care coordinators.
• Stay current with medical developments, clinical guidelines, and continuing education requirements.
• Adhere to compliance standards, ethical practices, and all applicable healthcare regulations.
Qualifications
MD or DO from an accredited medical school
Board certified or board eligible in Family Medicine or Internal Medicine
Active or eligible for unrestricted medical licensure in CT
Commitment to clinical excellence and delivering patient-centered care
Working Hours
Weekdays only
Limited phone call only (~2 weekends per year, 1 weeknight ~2 months)
