Skilled Nursing Facility (SNF) Physician (New grads are welcome)
Location: Fort Collins, CO
Employment Type: Full-Time (3-4 days/week)
Compensation: $110 to $120/hr. (on-call compensation paid separately) (sign-on or relocation assistance)

Position Summary
We are seeking a dedicated and compassionate SNF Physician to deliver high-quality medical care to residents within skilled nursing facilities across Northern Colorado. This position is ideal for a physician looking for a balanced schedule, autonomy in clinical decision-making, and a strong support system.
Key Responsibilities
· Deliver comprehensive medical care to residents in skilled nursing facilities.
· Work 3-4 days per week, with 10-hour shifts. No requirement to stay onsite after duties are completed.
· Participate in 3-4 on-call shifts per month, including nights and weekends.
· Collaborate with a medical assistant team to manage scheduling, triage, faxes, prior authorizations,
· Ensure timely and accurate documentation in compliance with facility and regulatory standards.
Qualifications
· Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
· Board-certified or board-eligible physician.
· Current and active BLS certification.
· Proven clinical experience, ideally in SNF, geriatrics, or post-acute care settings.
· Strong communication and collaboration skills.
· Commitment to delivering compassionate and patient-centered care.
