Responsibilities
- Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
- Drive high-quality, safe, and cost-effective patient care
- Collaborate with interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Provide oversight, guidance, and mentorship to clinicians within facilities
Position Details
- Schedule: Monday through Friday
- Patient volume: 20 patients per day
- Qualifications
- Background in providing primary care in internal medicine (IM), family medicine (FM), emergency medicine (EM), urgent care, or PM&R
- Ability to work flexible schedules
