Family Practice
Primary Care clinician
MUST BE able to see ALL AGES
MUST BE available for a minimum of 3days per week.
prefer Tues/Wed/Thrs if only 3days
Volume: 20 patients per day
Coverage location: Conyers
Monday - Friday
8:30-5:30
Dates of Coverage
End of March -ongoing
Please list all dates provider can work.
Epic Training is every other week 8-4:30p. Epic training can only happen after credentialing, and must be completed before the provider can start.
