Annashae is seeking Primary Care provider for a mental health facility near Sparta, IL. This is a full-time position & hours are Monday-Friday 8a-430p with no weekends, no call & no Holidays. We provide malpractice insurance & can credential quickly.
Additional Duties include, but are not limited to:
- Provide treatment to a caseload of patients on the pavilion
- Evaluate assigned cases at intake
- Sign certificate of admission and testify in court
- Order and interpret lab tests
- Conduct initial physical exams(H&P) and makes diagnoses of patients
- Develop and implements treat plan
- Follow up exams and documents changes in progress notes
- Prescribe and direct administration of medicines
- Monitor effectiveness of medication

Requirements:
- Current unrestricted Illinois License
- Board Certified/Eligible in Internal or Family Medicine
- Current Illinois Controlled Substance certificate
- Current unrestricted DEA certificate
- Mantoux skin test prior to starting work or documentation of negative Mantoux within last 90 days
- Clearance of required fingerprints and drug screen
