Locums – Eureka, California, Primary Care Job Clinical Role:

    • Start Date: 7/15/2024
    • Ongoing needed
    • Patients Per Shift: 16
    • Tuesday - Friday 8-5
    • Coverage Type: No Call
    • EMR System: Epic
    • Board Certified – Required
    • BLS - Required
    • Active CA Liscense – Required
    • Acute Care Clinic Opportunity (Urgent Care)
    • .8 FTE, 4 days/week (Tues thru Fri), Wed on Mobile Van (only see adults)
    • Mental health and Substance Abuse Care exp – Required
    • Urgent Care/FM (Urgent care)/IM (Urgent care)
    • Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for
    • Includes some Chronic Pain Management
    • Chart closure within 72 hours
    • Schedule = 28 PPD in UC clinic, minimum see 17 PPD on Mobile Van
    • A mix of peds/adults (Will consider IM only)
    • A decision is typically made within 7-14 days
    • Will accept DO
    • BC is required
    • Active DEA needed before the start
    • Credentialing takes 30-90 days (six weeks is common if the provider submit app./supporting documents timely)
    • The following immunizations are required for credentialing (before start date): 2022-2023 Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years)
    • Primary series plus booster, regardless of the date received or 1 dose of the current COVID vaccine (2023/2024 formulation)
    • HCO & Vendor agree to a 30-day notice for ending all assignments
