Medical Director (FP, IM) – Post Acute Care

Seeking an experienced Board-Certified Family Medicine or Internal Medicine physician to join a practice providing primary care to adults in Long Term Care facilities.

Full-time position

No primary night call.

If you live in Arizona, south of Phoenix, in Green Valley, Rio Rico, or Sahuarita, you are in the perfect location to support our communities near Tucson. Enjoy a flexible work model with a mix of onsite rounding and remote work.

Job Duties:

• Primary care rounding in SNF and Facility Medical Directorship.
• Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care.
• Collaborate with clinicians, facility leaders and others regarding care and services.
• Build relationships with your patients, patients’ family members, and facility staff to facilitate effective treatment delivery.
• Conduct clinical case reviews, as needed and share tools, information, and guidelines as they relate to quality of care.
• Collaborate with leadership to review care coordination initiatives, develop collaborative intervention plans, and share ideas about network management issues.

Will not accept visa candidates. Candidates must have at least 1 year post residency or fellowship experience.
