Seeking a physician who is:
- Board certified in internal medicine (IM) or family medicine (FM).
- Passionate about serving older adults.
- Enjoys working with patients in all levels of post-acute care including LTAC, sub-acute rehab, and inpatient rehab.
Responsibilities:
- Work alongside pharmacists on medication management.
- Complete Medicare required visits.
- Complete acute, follow up, and discharge visits as necessary.
- Implement an individualized plan of care for patients' goals.
- In collaboration with the IDT, determine transitional care.
- Make recommendations on DME, home health services, or outpatient services after a short stay.
