Practice/Organization Details:
Provide exceptional care to patients in a hybrid in-home and virtual model offering flexibility and supreme work/life balance
Manage a low-volume patient panel of high complex chronic and acute medical patients and develop individualized care plans
Oversee a clinical care team including dedicated APPs, pharmacist, nursing, and social worker support, practicing in a collaborative and patient-centered approach
Flexible schedule to work around your needs throughout the day
Leadership potential as you have a chance to help build and establish program as it continues to grow
Do away with RVU's and enjoy a strong base salary with outcome-based bonuses - earning potential of over $350,000
