We are seeking a compassionate and dedicated healthcare provider (Physician or Nurse Practitioner) to join our client’s house call program, delivering comprehensive medical care to patients in the comfort of their own homes. As a House Call Provider, you will play a vital role in addressing the unique healthcare needs of homebound patients, promoting continuity of care, and enhancing their quality of life through personalized and patient-centered services.

Responsibilities:

  • Conduct initial assessments and comprehensive medical evaluations of homebound patients, including physical examinations, medical history review, medication reconciliation, and functional assessments, to establish accurate diagnoses and treatment plans.
  • Provide timely and evidence-based medical care, diagnosis, and treatment of acute and chronic health conditions, managing complex medical issues and coordinating care with specialists and ancillary services as needed.
  • Develop individualized care plans tailored to patients' preferences, goals, and functional abilities, integrating input from patients, caregivers, and interdisciplinary team members to optimize outcomes and promote patient autonomy.
  • Perform routine and preventive healthcare services, including vaccinations, health screenings, medication management, and chronic disease management, to prevent illness, manage symptoms, and improve overall health and well-being.
  • Educate patients and their caregivers about their health conditions, treatment options, medication adherence, lifestyle modifications, and strategies for maintaining functional independence and maximizing quality of life at home.
  • Coordinate with home health agencies, durable medical equipment providers, and community resources to facilitate access to supportive services, medical supplies, and assistance with activities of daily living, ensuring a safe and supportive home environment for patients.
  • Conduct follow-up visits and ongoing monitoring of patients' health status, treatment response, and adherence to care plans, adjusting interventions as needed to address changing needs and optimize health outcomes.
  • Document patient encounters accurately and thoroughly in electronic health records (EHR), maintaining confidentiality and compliance with regulatory standards and organizational policies.
  • Collaborate with interdisciplinary team members, including registered nurses, social workers, physical therapists, and other healthcare professionals, to ensure coordinated and holistic care delivery that addresses patients' medical, social, and psychological needs.
  • Participate in regular case conferences, team meetings, and quality improvement initiatives to review patient cases, discuss best practices, and enhance the effectiveness and efficiency of house call services.
  • Uphold ethical principles, professional standards, and cultural competence in all interactions and interventions, demonstrating empathy, respect, and sensitivity to patients' diverse backgrounds and individual preferences.

Requirements:

  • Physician: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school. Nurse Practitioner: Master's or Doctoral degree in Nursing from an accredited program.
  • Current licensure and board certification as a Physician (MD/DO) or Nurse Practitioner (NP) in the state of practice, with prescriptive authority as applicable.
  • Minimum of 2-3 years of clinical experience in primary care, internal medicine, family medicine, or geriatrics, with experience in home-based care or care of homebound patients preferred.
  • Strong clinical assessment, diagnostic reasoning, and treatment planning skills, with proficiency in managing acute and chronic medical conditions in diverse patient populations.
  • Excellent communication and interpersonal skills, with the ability to establish rapport, build trust, and communicate effectively with patients, families, and interdisciplinary team members.
  • Compassion, empathy, and cultural sensitivity in providing patient-centered care to individuals with complex medical needs and functional limitations.
  • Ability to work independently with minimal supervision, prioritize tasks, and manage time effectively in a dynamic and unpredictable home care environment.

Please note- The annual base salary range is $140k to $230k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
