Payment Details:
Hourly Pay Rate: $170
Hourly On Call Rate: $20
Hourly Call Back: $170

Shift Schedule:
- 32 hours per week for 3 months (Four 8-hour shifts)
- Potential adjustment to 16-20 hours per week (Two-to-Three 8-hour shifts) after 3 months (could continue at 32 hours per week)
- On Call Requirement will be as coordinated with Medical Director

Physician Qualifications:
1. Be licensed to practice as a physician in the State of Vermont.
2. Board certification required.
3. Be a member of the medical staff of a general hospital and/or medical practice.
4. Has at least 5 years experience working with Veterans and Geriatrics, preferably in a nursing home setting.

Physician Responsibilities:
1. The coordination of medical care at the Home and ensuring the proper medical and health-related services are provided to all assigned residents.
2. Advise the Medical Director on conditions that might affect the health or welfare of the residents and advise on prospective admissions.
3. Have working knowledge of, adhere to, and assist the facility in maintaining compliance with state and federal regulations specifically regarding, but not limited to medical care and services, resident rights, quality of care, abuse, and neglect.
4. Review, evaluate, and recommend revisions to all assigned residents’ treatment and care plans.
5. Ensure that all residents are seen and an initial history and physical are completed within 48 hours of admission.
6. Participate in Quality Assurance Performance Improvement (QAPI) Performance Improvement Projects as directed by the Medical Director and/or facility administration.
7. Establish a regular schedule of facility visits.
8. Participate in quarterly medical staff meetings with the facility administration and other providers.
9. Establish coverage for after-hour emergencies, holidays, vacations, and for any other reason you will be unavailable to facility staff for emergency consultations and notifications.
10. Complete ten (10) hours of annual education on caring for geriatric/veterans in the nursing home setting annually, at your expense.
11. Be able to access the facility’s Electronic Medical Record system off-site, facility will arrange for access via password.
12. Be a liaison between the facility and various community health care providers, as needed, to ensure the appropriate care and services are provided to your assigned caseload when they are outside of the facility.
13. Participate in care plan/family meetings as needed.
14. Provide staff education as deemed appropriate by the Medical Director
and/or Facility Administration.
15. Will use their resources to bill residents’ insurance including but not
limited to Medicare, Medicaid (VT), commercial insurance, and private pay.
