Position Overview: We are seeking a dedicated and compassionate Internal Medicine Physician to join our team at a prestigious FQHC in Dorchester, MA. As a primary care provider within our Patient-Centered Care Model, you will have the opportunity to serve a diverse panel of patients, providing comprehensive outpatient care. Key Responsibilities:

  • Serve as a primary care provider, delivering comprehensive outpatient care to a panel of patients within the Patient-Centered Care Model.
  • Assess health risks, promote disease prevention, and provide evidence-based medical care to patients of all ages.
  • Conduct thorough medical evaluations, diagnose and treat acute and chronic medical conditions, and develop individualized care plans in collaboration with patients and interdisciplinary team members.
  • Utilize electronic health record (EHR) systems to document patient encounters, maintain accurate medical records, and ensure compliance with regulatory standards.
  • Participate in multidisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery and promote clinical excellence.
  • Collaborate with ancillary staff, including nurses, medical assistants, and support personnel, to optimize patient care coordination and workflow efficiency.

Qualifications:

  • Completion of an accredited residency program in Internal Medicine.
  • Current Massachusetts MD or DO license, in good standing.
  • Board certification or eligibility in Internal Medicine or Family Practice.
  • Strong clinical skills and proficiency in evidence-based medicine.
  • Excellent communication, interpersonal, and problem-solving abilities.
  • Commitment to providing culturally competent and patient-centered care.
  • Ability to work effectively within a multidisciplinary team environment.
