Internal Medicine Opportunity - Rochester, MN

Olmsted Medical Center has a 1.0 FTE outpatient Internal Medicine opportunity for you to join our multi-specialty team at our Rochester Southeast Clinic. This shared position would assume an established practice joining other clinicians in an outpatient clinic and in serving our geriatric patients for post-acute/long-term care. Experience in palliative care preferred.
  • Clinic:
    • Outpatient clinic, no call, 8am-5pm, Monday - Friday schedule
    • Supported by a 24x7 hospitalist group
    • Care Coordinators
    • Over 35 specialty services
    • Epic
  • Post-Acute/Long-Term Care:
    • Partake in nursing home call
    • Assist with care planning, admissions, and facility discharges
    • Clinic visits focus on:
      • Brain Health/Memory Care Clinic
      • Palliative Care Clinic
      • Medical Cannabis Clinic
      • Fall Prevention Clinic
      • Acute Care needs
      • Opportunity for medical directorship in long-term care facilities
