Internal Medicine Physician – Post Acute Care

Join our practice as a Board-Certified Internal Medicine physician, focusing on post-acute care across various settings including LTAC, sub-acute rehab, and inpatient rehab.

Position Details:

• Full-time position offering flexibility with a self-set schedule, Monday to Friday.
• Collaborate with a multidisciplinary team including pharmacists for comprehensive medication management.
• Conduct Medicare-required visits, acute, follow-up, and discharge visits as needed.
• Develop individualized care plans aligned with patients' goals in partnership with the Interdisciplinary Team (IDT).
• Provide recommendations for DME, home health services, or outpatient services post-short-stay.

Requirements:

• Board certification in Internal Medicine.
• Not accepting visa candidates.
