The Physician/Medical Director is responsible for clinical direction and oversight of Healthcare Providers in various post-acute facilities, providing in-patient holistic medical services utilizing innovative technologies to improve patient access impacting quality outcomes. This role will oversee 3-7 facilities and round Monday-Friday and will not be responsible for any call during the evening, weekend, or holidays.

These medical services include:

  • Planning, directing, and evaluating resident/patient care
  • Examination and treatment of chronic and acute episodic illnesses; and treatment of minor injuries, accidents, and other injuries/illnesses.
  • Performs elected laboratory testing, writes prescriptions, and diagnoses and treats medical conditions within the scope of their license expertise, protocols, and available equipment.

Job Responsibilities

Works with the Medical Team to provide patients with primary medical care services within the standards of practice and care for facility patients/residents.
Acts as a resource person for decisions, planning, and policies affecting the health of residents/patients. * Exercises professional judgement and provides consultation to Providers concerning appropriate treatment of residents/patients.
Collaborates with Physician Assistants, Nurse Practitioners, and facility staff to identify resident/patient needs and helps develop plans to meet these needs.
Follows residents/patients in a post-acute skilled nursing facility, inpatient rehabilitation, assisted living, and other post-acute care facilities, as needed for continuity of care.
Assists Providers with emergency treatments and more complex procedures.
Adheres to facility policies, procedures, and objectives; ongoing quality improvement objectives and safety; and environmental and infection control standards.
Serves as a member of committees dealing with health care, to include Quality Assurance/Performance Improvement (QAPI), and Antibiotic Stewardship, etc.

Qualifications

Minimum of two (2) years’ experience in a primary care physician role.
M.D. or D.O. degree from an accredited United States medical school.
Successful completion of a residency program at an accredited healthcare institution.
Board certification in primary care or related field from said residency. (Preferred)
Current license in the State of Illinois
Current Drug Enforcement Agency (DEA) and Department of Public Safety (DPS) registration for prescription writing
