The Practice: AgeWell is an independent (provider-owned) practice comprised of five providers (one MD and four NPs) providing primary care to seniors – adults 60 and older, with Medicare or Medicare Advantage. Our patients represent the whole spectrum of aging, from well seniors to those with multiple chronic conditions. Our north star is patient centered care. We help our patients achieve their health goals. We are committed to the principles of the Patient Centered Medical Home and have been recognized as such since 2011.

Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

We are located in Colorado Springs, CO where the outdoor recreation options are endless, and the local communities are safe and uniquely beautiful. The Position:

  • 1.0 FTE opportunity – 32 patient facing hours/week
  • See an average of 8-10 patients/day – appointments are 30 minutes minimum
  • NCQA Level 3 patient centered medical home model – support includes a dedicated MA and transitional care RN
  • Opportunity to build ambulatory care panel with no hospital call responsibilities
  • EMR – Aprima, utilized since 2012

Qualifications:

  • MD or DO degree from fully accredited medical or osteopathic school of medicine
  • Successful completion of Internal Medicine, Family Medicine or Geriatrics residency program
  • Board Certification or Eligibility in corresponding specialty
  • Physician must meet employer credentialing standards
