Pinnacle Care Providers / CatenaCare

Our physicians provide high-quality care in skilled nursing, long-term care, assisted living, and senior living communities. The physician is a clinical leader within Pinnacle's embedded postacute model, working closely with APPs, Care Coordinators, facility teams, patients, families, and other care partners.

The role focuses on continuity, proactive management of complex older adults, care coordination, appropriate utilization, and strong facility partnership. Physicians may also serve as Medical Directors for assigned skilled nursing facilities.

Job Responsibilities

Clinical Care

• Provide scheduled onsite rounds, evaluations, follow-up care, acute visits, and chronic
disease management at assigned facilities.
• Oversee medically complex and high-risk patients and maintain appropriate plans of
care.
• Manage common geriatric and post-acute conditions across acute and chronic care
needs.
• Identify changes in condition early and help prevent avoidable emergency department
visits and hospitalizations when appropriate.
• Provide timely follow-up after hospitalizations, emergency visits, changes in condition,
and care transitions.
• Participate in goals-of-care and advance care planning discussions.
• Support medication reconciliation, appropriate prescribing, and deprescribing when
indicated.
• Coordinate care with specialists, hospice, therapy, behavioral health, pharmacy, and
other clinical partners.

Interdisciplinary Care Model

• Collaborate with Pinnacle APPs and Care Coordinators on patient identification,
scheduling, follow-up, and care-gap closure.
• Use facility rosters, visit cadence, quality reports, and clinical data to prioritize care.
• Follow Pinnacle's standardized clinical workflows and protocols
Support Annual Wellness Visits, Advance Care Planning, chronic care programs, and
other value-based services when appropriate.
• Participate in patient attribution, care management, risk adjustment, quality, and valuebased care initiatives.

Facility Partnership

• Maintain a dependable presence and strong relationships with facility administrators,
nursing leaders, and clinical teams.
• Communicate promptly regarding changes in condition, clinical concerns, care plans,
and treatment recommendations.
• Participate in quality, interdisciplinary, and facility meetings as appropriate.
• Partner with Pinnacle clinical and account management leaders to address concerns
and strengthen facility satisfaction.

Medical Directorship
• Physicians may serve as Medical Director for one or more skilled nursing facilities.
Responsibilities may include:
• Provide clinical leadership and participate in Quality Assurance and Performance
Improvement activities.
• Collaborate with facility administration, nursing, pharmacy, infection prevention, therapy,
and other departments.
• Support clinical policies, standards of care, and review of quality trends.
• Support initiatives related to hospitalizations, readmissions, antibiotic stewardship,
infection prevention, falls, and medication management.
• Support practitioner performance and participate in required Medical Director and facility
meetings.
• Medical Director duties are performed in accordance with facility agreements and
applicable regulatory requirements.
Documentation, Coding, and Compliance
• Complete accurate documentation within Pinnacle timelines and follow onsite
documentation and dictation expectations.
• Ensure documentation supports medical necessity, diagnoses, complexity, services, and
appropriate coding.
• Participate in compliance and coding education, chart audits, and follow-up coaching.
• Maintain required licenses, credentials, training, and compliance with applicable CMS,
payor, state, federal, and facility requirements

Qualifications

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Active, unrestricted medical license in the applicable state; board certified or board
eligible in an appropriate specialty.
• Active DEA registration where applicable and eligibility to participate in Medicare,
Medicaid, and applicable payor programs. Ability to obtain required facility and payor
credentialing.
• Strong clinical judgment, communication skills, and ability to manage medically complex
older adults within an interdisciplinary team.

Preferred Qualifications
Experience in geriatrics, internal medicine, family medicine, post-acute care, skilled
nursing, or long-term care, Experience working with APPs and/or serving as a Medical Director, Experience with CMS quality programs, value-based care, population health, quality improvement, or utilization management

Working Hours

M-F - some on-call during week days
