Seeking a compassionate and experienced Primary Care Physician to provide comprehensive medical care to residents in assisted living communities throughout the Albany region. This role offers a flexible schedule, established patient base, supportive administrative resources, and the opportunity to focus on delivering patient-centered care to an aging population.
Job Responsibilities
• Evaluate, diagnose, and manage acute and chronic illnesses
• Develop and implement individualized treatment plans
• Prescribe medications and monitor ongoing care
• Order and interpret diagnostic tests
• Coordinate care with specialists, facility staff, and family members
• Maintain accurate and timely EMR documentation
• Promote preventive health measures and chronic disease management
Qualifications
• Active New York Medical License
• Active DEA Registration
• Medicare and Medicaid Credentialing Required
• Minimum 2 Years of Primary Care Experience
• Strong Clinical and Communication Skills

Working Hours
• 4–5 Days Per Week
• Travel Between Hoosick Falls, Cambridge, and Troy, NY
