Geriatric Medicine Physician Pod Leader - Low Volume PACE - North Hollywood, California

Primary Care Physician with focus on geriatric, community based, value based care

Full-time employed position in an outpatient PACE model

Average of approximately 8 patients per day with no hospital rounding

Regular schedule Monday through Friday, 8 a.m. to 5 p.m., including some home visits

Rotating phone only call a few nights or weekends per month

Perform comprehensive in person assessments and lead the interdisciplinary team to create care plans

Coordinate discharges and placement from acute, short term, and long term care settings

Emphasis on interdisciplinary team based care for Medicare and Medi Cal eligible seniors

Competitive base salary in the range of $275,246 to $330,595plus quarterly bonus eligibility

MD or DO with completion of an ACGME accredited residency and doctorate level degree

Active unrestricted state medical license, state controlled substance registration, and DEA required; CPR and First Aid required within 180 days of hire

Board certification preferred in Internal Medicine, Family Medicine, Emergency Medicine, or Geriatrics with 2 to 5 years of primary care experience in geriatrics preferred

Located in North Hollywood, California near attractions including the NoHo Arts District, Universal Studios Hollywood, and Griffith Park

Approximately 12 miles (about 25 minutes depending on traffic) to downtown Los Angeles and 25 miles (about 40 to 50 minutes) to Santa Monica and Pacific Coast beaches
