NURSING HOME PRIMARY CARE/GERIATRICS PHYSICIAN ROUNDING OPPORTUNITY
COMMUNITY
Metro Los Angeles and Los Angeles County
Nursing Home Rounding Physician – Los Angeles, CA
We are seeking a dedicated and compassionate Nursing Home Physician to provide ongoing medical care for residents across one or more skilled nursing facilities in the Los Angeles area. The physician will conduct regular rounds, manage acute & chronic conditions, collaborate with interdisciplinary teams while ensuring high-quality, patient-centered care for an aging population with complex medical needs
ROLE OVERVIEW
• Begin @ Skilled Nursing Facilities; can add Long Term Acute Care when ready
• Primarily weekends will be off. Physicians make own schedule
• Your care team’s Mid levels also cover each of the facilities. Physicians supervise Mid- levels. Physicians see admits within 72 hrs to stay in compliance
• Cover multiple facilities each week under the model of ‘Round and Go’
• Cover multiple facilities each week under the model of ‘Round and Go’. Physicians choose 1 facility to visit every day. IF there is an emergent need for a patient to be seen or a difficult family then will be added to calendar ASAP
• CALL: Mid levels cover all phone calls/ text messages. MD/DO’s only called when an absolute emergency. Low patient panel
• Communicate and provide timely reports to private practice leadership as needed
DUTIES
• Conduct routine + as-needed rounds on SNF or LTAC residents, ensuring timely evaluation & follow-up
• Manage acute medical issues, chronic disease progression & post-hospitalization care
• Perform comprehensive assessments, including history, physical examination, medication review, and care planning
• Coordinate care with nursing staff, therapists, social workers, case managers & facility leadership
• Communicate effectively with families + caregivers regarding patient status, goals of care & treatment options
• Review + sign off on orders, labs, imaging where available & therapy recommendations
• Ensure compliance with state, federal & facility regulations, including documentation standards
• Participate in quality improvement initiatives, readmission reduction efforts & interdisciplinary meetings where available
• Provide guidance on advance care planning, palliative care needs, and transitions of care

Qualifications
• Board‑certified or board‑eligible in Internal Medicine, Family Medicine, or Geriatrics
• Active California medical license (or ability to obtain)
• Experience in skilled nursing facilities, long‑term care, geriatrics, or post‑acute care preferred
• Strong communication, collaboration & clinical decision‑making skills.
• Ability to work independently across multiple facilities.
