Internal Medicine/Family Medicine or Geriatrics Physician – Post-Acute Care | Philadelphia, PA | Flexible Rounding
Opportunity Highlights:
• Full-time (5 days/week) post-acute care position in skilled nursing facilities
• Provide primary care rounding and manage complex patient cases
• Collaborate with interdisciplinary teams including clinicians and facility leadership
• Build relationships with patients, families, and care teams to improve outcomes
• Mentor and provide oversight to nurse practitioners
• Participate in care coordination and quality improvement initiatives

Compensation & Benefits:
• Estimated annual compensation: $260,000 – $310,000+ with uncapped earnings

Qualifications:
• Board Eligible/Certified in Internal Medicine, Family Medicine, or related field
• Active Pennsylvania license (or in process)
• Active DEA (or in process)
• Minimum of 1 year experience preferred
• Post-acute experience preferred but not required
