Job description
A PACE healthcare program in Cambridge, Massachusetts is seeking experienced Primary Care providers including Physicians, Nurse Practitioners, and Physician Assistants to deliver comprehensive care to elderly patients with complex medical needs.
This opportunity allows providers to work in a coordinated, patient-centered care model that focuses on improving quality of life for medically fragile seniors. Providers collaborate closely with interdisciplinary teams to deliver holistic and preventive care in outpatient, home, and nursing facility settings.
The role offers a structured weekday schedule with no call responsibilities and the opportunity to build meaningful relationships with patients through lower patient panel sizes.
Job Responsibilities
โข Manage chronic and acute medical conditions
โข Conduct clinic visits, home visits, and nursing facility visits as required
โข Participate in interdisciplinary care team meetings
โข Coordinate patient care with nurses, therapists, and social workers
โข Manage patient communications including phone calls and in-basket coverage
โข Document patient encounters using Epic EMR
โข Develop individualized care plans for complex elderly patients
Qualifications
โข Certified Nurse Practitioner or Physician Assistant (for APP candidates)
โข Board Certified (Physicians)
โข Active Massachusetts Medical License required
โข Experience caring for geriatric or complex patient populations preferred
โข Flu vaccination required (no waivers accepted)

8:00 AM โ 5:00 PM
