Job description Medical Director – Geriatrics

Location: West Springfield, Massachusetts Position Type: Full-Time Schedule: Monday – Friday

Position Overview

Seeking a Medical Director to lead a Program of All-Inclusive Care for the Elderly model serving adults 55+ who qualify for nursing home level of care but are able to safely live in the community at enrollment.

This program provides the full continuum of medical and supportive services, focusing on maintaining independence and coordinating care across settings, including nursing homes when needed.

Reports To: Executive Director Administrative Supervision: Executive Director

Schedule
  • Full-Time

  • Monday – Friday

  • Participation in provider on-call rotation as required

Licensure & Certification Requirements (Hard Stops)
  • Active Massachusetts Physician License – Required

  • DEA Registration – Required

  • Board Certification – Internal Medicine or Family Medicine – Required

  • Board Certification in Geriatrics – Preferred

  • Minimum 1 year experience with frail/elderly population – Required

  • Previous Supervisory Experience – Required

  • Experience/Additional Qualifications in Geriatrics – Preferred

Leadership & Clinical Responsibilities Medical Oversight
  • Provide general medical direction and supervision of participant care

  • Responsible for clinical outcomes and Quality Assurance Performance Improvement (QAPI) oversight

  • Develop and maintain medical policies, procedures, standards, and protocols

  • Supervise PCPs and APPs

Direct Patient Care
  • Conduct H&Ps for new participants

  • Perform periodic and event-based reassessments

  • Develop and manage treatment plans

  • Order and evaluate labs/diagnostic testing

  • Provide care across participant living settings

  • Manage chronic disease in frail older adults

Interdisciplinary Collaboration
  • Participate in intake and assessment decisions

  • Engage in interdisciplinary team meetings

  • Lead and participate in family conferences

  • Educate staff, participants, and families

  • Collaborate in nursing treatment plans

  • Help establish and achieve operational goals

Ideal Candidate Profile
  • Strong leadership and supervisory background

  • Comfortable in a collaborative, interdisciplinary care model

  • Experience managing complex chronic conditions in elderly patients

  • Interested in an alternative, community-based care setting

  • Able to manage shifting priorities based on participant needs
