Job description
Provide compassionate, proactive medical care across multiple post-acute and long-term care facilities in Maine. Serve as attending physician for 4–5 facilities, collaborating with interdisciplinary teams and supported by a Medical Director. Structured daytime schedule with strong work-life balance.
Key Responsibilities:
- Serve as attending physician across assigned facilities
- Complete admissions, medication reconciliation, and transitional care documentation
- Perform weekly follow-up visits and acute condition assessments
- Complete discharge visits when needed
- Partner with nursing and leadership teams to ensure coordination and communication
- Support dementia care, wound care, chronic disease management, prevention, and infection control
- Participate in interdisciplinary meetings and quality initiatives
Qualifications:
- MD/DO with active Maine license or current IMLC license
- Experience in Family Medicine, Internal Medicine, Geriatrics, or PM&R preferred
- Current Geriatric experience is a must (post-acute or geriatric care experience strongly valued)
- Board certification/board eligible preferred, non-boarded Physicians considered as well
- Ability to work independently and collaboratively
- Excellent communication skills
- EHR and telemedicine proficiency
Additional Details:
- Pay Rate: $150 per hour
- Schedule: 8 AM – 5 PM, No Call
- 20 PPD Minimum
- ME License or IMLC Compact License Accepted

Typical week:
2 days at brewer
1 Westage (possible to work at East side on the same day, vice versa)
