ATC West Healthcare Services is seeking an experienced Geriatric Medicine Physician to provide locum coverage at a facility in Hampden County, MA. The position combines clinical duties, interdisciplinary collaboration, participant/family education, and program leadership to support high-quality, patient-centered care. This role is ideal for a physician experienced in geriatrics and chronic disease management who thrives in a collaborative, community-based care environment.

Schedule / Dates

  • Schedule: Monday–Friday

  • Hours: Full-time

Job Details & Responsibilities

  • Conduct histories and physicals for new participants, as well as event-based and periodic reassessments.

  • Develop medical treatment plans and provide medically necessary services across participant care settings.

  • Interview and evaluate applicants and caregivers for PACE program eligibility and participate in Intake and Assessment decisions.

  • Discuss medical conditions, prognosis, and care planning with participants and caregivers.

  • Work collaboratively within TH PACE’s shared, interdisciplinary culture.

  • Collaborate with nursing staff on participant treatment programs.

  • Contribute as part of the leadership team to achieving operational goals.

  • Actively participate in interdisciplinary team meetings, educating team members on diagnoses, functional status, and treatment options.

  • Participate in team and family conferences and shared decision-making.

  • Educate participants and families on health maintenance and chronic condition management.

  • Supervise health care and treatment plans developed by the interdisciplinary team.

  • Perform physical exams; order, review, and document laboratory and diagnostic testing.

  • Participate in provider on-call rotation.

  • Manage evolving priorities based on participant needs.

  • Demonstrate strong skills in physical assessment and chronic disease management for frail older adults.

  • Contribute to a creative, alternative health care model.

Required Skills & Qualifications

  • Current and active Massachusetts plenary physician license (Required)

  • Board Certification in Internal Medicine or Family Practice (Required)

  • Board Certification in Geriatrics (Preferred)

  • Experience or additional qualifications in geriatrics (Preferred)

  • DEA registration (Required)

  • Previous supervisory experience (Required)

  • Minimum 1 year of experience working with frail or elderly populations (Required)

  • Certifications Required: Board Certification, CSR/CDS (Controlled Substance), DEA

  • Ability to effectively manage chronic diseases in older adults

  • Strong communication, interdisciplinary collaboration, and patient/family education skills

  • Ability to manage changing priorities in participant-centered care environments
