π Position Overview
A full-time Medical Director is needed to lead a comprehensive community-based senior care program. This is a leadership role focused on delivering high-quality, coordinated care for seniors with chronic conditions, emphasizing independence, wellness, and dignity.
You will oversee clinical operations, guide interdisciplinary care teams, supervise providers, and ensure best practices in geriatric medicine are upheld.
π©Ί Key Responsibilities

Conduct H&P exams for new and established participants
β
Develop and implement medical treatment plans
β
Participate in intake evaluations and care planning
β
Provide medical guidance to participants and caregivers
β
Collaborate with nursing and interdisciplinary teams
β
Oversee QAPI and clinical quality standards
β
Supervise PCPs and APPs
β
Participate in family conferences and team meetings
β
Manage chronic disease care for frail older adults
β
Participate in shared on-call rotation
β
Maintain medical policy standards and protocols

β Required Qualifications
- Active Massachusetts medical license
- Board Certified: Internal Medicine or Family Medicine
- Geriatrics Board Certification or geriatric experience preferred
- CDS Certification & DEA required
- Minimum 1 year experience with frail or elderly patients
- Strong skills in chronic disease management
- Proven ability to supervise clinical staff
- Must be able to manage evolving priorities
