Primary Care Physician – PACE Program

Our client is seeking a dedicated Primary Care Physician to provide comprehensive, compassionate care for participants enrolled in their PACE (Program of All-Inclusive Care for the Elderly) program. This position plays a key role in promoting participants’ health and independence by coordinating medical care and collaborating with an interdisciplinary team (IDT) to ensure the highest quality of care.

Job Responsibilities

Position Highlights:

Deliver comprehensive, coordinated medical care guided by the PACE benefit model.
Conduct initial, semiannual, annual, and periodic participant assessments.
Partner with the interdisciplinary team to develop, review, and implement individualized care plans.
Oversee diagnostic testing, medication management, and appropriate specialist referrals.
Provide follow-up care and collaborate with participants’ families and support networks.
Participate in team meetings, care conferences, and family discussions as needed.
Coordinate care for hospitalized and nursing home participants, ensuring smooth transitions and adherence to care plans.
Participate in quality assurance and performance improvement programs (QAPI).
Document all care in eClinicalWorks (EMR) and maintain compliance with all policies, procedures, and HIPAA regulations.
Additional Responsibilities:

Offer medical guidance and leadership to the IDT and collaborate with clinic leadership on site-level medical operations.
Participate in after-hours telephonic coverage on a rotating basis.
Contribute to committees and initiatives such as infection control, ethics, and quality improvement.
Maintain a team-oriented approach to patient care and collaboration.

Qualifications

Board Certified or Board Eligible Physician in Internal Medicine or Family Medicine.
Experience with geriatric or complex care populations preferred.
Strong communication, collaboration, and documentation skills.
Commitment to patient-centered, interdisciplinary care.

Working Hours

Mon-Fri 8 am - 5:30 pm
