Pay Range: $220k - $250k Annual

Overview: We're seeking a compassionate Primary Care Physician to join our SNF Transitions Team, delivering sub-acute and palliative care to geriatric patients. Ideal for physicians with hospitalist or SNF experience seeking a meaningful role with excellent work-life balance.
- Provide subacute/palliative care in SNF
- Lead care planning and IDT meetings
- Guide goals-of-care and advance care planning
- BE/BC Physician, CO license; active DEA
- 2+ years hospitalist/SNF/post-acute experience is required
- Strong in geriatrics, chronic disease, and palliative care
- Bilingual is a plus
