This Locum to Perm role is ideal for a provider looking for a long-term opportunity in an innovative and supportive care model for aging adults.
Job Responsibilities
• Conduct H&Ps, periodic assessments, and manage chronic conditions
• Participate in intake assessments and interdisciplinary team meetings
• Supervise and guide APPs and PCPs
• Lead clinical quality and performance improvement initiatives
• Maintain medical policies, protocols, and care standards
• Participate in on-call rotation
Qualifications
• Board Certified in Internal Medicine or Family Practice – REQUIRED
• Geriatrics Certification – Preferred
• DEA Registration – REQUIRED
• At least 1 year of experience with elderly/frail population
• Supervisory experience – REQUIRED
• Strong chronic disease management and physical assessment skills
