💵 Salary: $240,000 – $270,000 + Performance-Based Incentives
🌟 Key Responsibilities
📝 Conduct H&Ps, periodic reassessments, and develop medical plans
💬 Discuss care plans, diagnoses, and prognosis with participants and caregivers
👥 Participate in intake evaluations and family/team conferences
🤝 Collaborate with interdisciplinary teams on chronic disease management
🧠 Provide leadership and strategic input as part of the senior care model
💊 Order labs, diagnostics, and perform physical exams
📞 Participate in on-call rotation (as required)
🎓 Educate participants, families, and team members on treatment options
🧓 Care delivered across multiple settings where participants reside
👨⚕️ Ideal Candidate Profile
✅ Active FL Physician License – Required
✅ Board Certified in Internal or Family Medicine – Required
🎯 Board Cert. in Geriatrics – Preferred
📄 CDS & DEA Registration – Required
🧠 Supervisory/Leadership Experience – Required
👵 1+ Year of Experience with Elderly or Frail Populations – Required
💡 Strong skills in physical assessment & chronic condition management
