Job Responsibilities
Capture and recapture of Hierarchical Condition Categories (HCCs)
Close any open care gaps through Point-of-Care (POC) testing
Complete and sign all chart notes
Qualifications
Training or interest in geriatrics welcome, but not required.
Current DEA license.
Board Eligible or Board Certified in Family or Internal Medicine.
California licensed physician or eligible to obtain CA license.
