Overview:
Are you seeking a flexible Monday–Friday schedule with no night call while delivering high-quality care in a post-acute setting? We’re hiring a dedicated physician to join a respected post-acute care practice in the Danbury, CT area. This full-time position offers schedule autonomy, excellent compensation, and the opportunity to work with a nationally recognized group.
Position Details:

Location: Danbury, CT

Setting: Post-acute care (skilled nursing and assisted living facilities)

Schedule: Full-time, weekdays only — no primary night call

Patient Volume: 15–20 patients/day

EMR: gEHRiMed
Key Responsibilities:
-
Deliver primary care to patients in post-acute care communities
-
Diagnose and manage chronic and acute conditions

Collaborate with interdisciplinary care teams, hospice, palliative, and home health services
-
Ensure compliant documentation and timely billing within Medicare guidelines

Contribute to transitions of care planning and patient-centered outcomes
Qualifications:
-
MD or DO; Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Emergency Medicine

Active or eligible Connecticut medical license

Prior experience in post-acute care a plus, but not required—training available
