The Opportunity

Post-acute medicine done as continuity rather than coverage. You round across sub-acute and long-term care sites spanning Burlington County, New Jersey and Philadelphia — Roosevelt, St. Joseph's Manor, Lafayette Redeemer, Saunders and Chestnut Hill — working alongside four to five nurse practitioners and physician assistants, seeing at least 16 encounters a day on a Monday-to-Friday schedule. These are patients you follow for months and years — which is the part of medicine that hospital work keeps taking away from people.

What You'll Own

• A continuity panel across SNF, long-term care and assisted living, rather than a rotating inpatient census.
• Clinical supervision of up to three APPs, within a team of four to five, who manage day-to-day and escalate to you.
• At least 16 encounters a day across a 12–16 patient panel, on flexible roughly 8-hour weekdays, plus one half-day clinic a week averaging 8 encounters.
• Sub-acute medicine with a 17–21 day average stay — long enough to see whether your plan worked. Weekend rounding 1-in-3 and beeper call one week in three.

Must-Haves

• MD or DO
• Board certified or board eligible in Internal Medicine, Family Medicine or Geriatrics
• Pennsylvania license, or eligibility in process
• Willingness to travel between sites

Nice to Have

• Geriatrics fellowship or substantial post-acute experience
• Prior APP supervision
• Point Click Care familiarity

What Working Here Is Actually Like

• $250,000 as a W-2 employee, or $280,000 as an independent contractor — your choice of structure.
• An additional $40 per encounter beyond the first 16 in a day.
• Full W-2 benefits. Medical, dental and vision from the first of the month after hire; employer-paid life at 1x salary up to $300,000; employer-paid long-term disability at 60% of salary; 401(k). The benefit plan year runs 1 October to 30 September.
• 40 hours of sick leave.
• Malpractice and tail both covered. Tail is the one people forget to ask about until they leave.
• Monday to Friday. Flexible roughly 8-hour days — no shift work, no nights.
• Point Click Care, purpose-built for post-acute rather than an acute EHR bent into shape.
• Physician-owned and physician-managed since 2012. When you escalate a problem, it reaches a physician who still works clinical shifts.
• Licensure is faster than you expect if you're coming from out of state. Pennsylvania participates in the Interstate Medical Licensure Compact, so if you already hold a qualifying license elsewhere, adding this one typically runs one to three weeks rather than months.

Straight Talk

This is a driving job as much as a clinical one — five sites across two states, every week, and a New Jersey license is required rather than optional. Ask us for the actual site list and the mileage policy before you decide, because the commute is the thing people underestimate. The work itself is steady and relational; the logistics are the cost.
