Join our robust team as a primary care provider for a San Diego based Program of All-Inclusive Care for the Elderly (PACE), providing high quality, compassionate geriatric care in an outpatient setting.

-Provides acute, chronic, preventive, curative and rehabilitative medical care to patients
-Manages transitions of care, chronic disease, advanced goals of care discussion and palliative management
-Refer participants to other in-house providers, including behavioral health
-Document visits in the electronic medical record
Hours
-Monday through Friday
-Full time
Preferred
• 1 year post completion of clinical training
• Employment experience in a community health center
• Experience working with elderly population
• Medical Spanish preferred
• Leadership experience a plus!
