The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.
The Medical Director provides general medical direction and supervision of the medical aspects of participant care. The Medical Director is responsible for the delivery of participant care, clinical outcomes, and oversight of the Quality Assurance Performance Improvement (QAPI) program. They are responsible for the development and maintenance of medical policies and procedures/standards and protocols. They provides direction and supervision to related Primary Care Providers (PCPs) and Advanced Practice Providers (APPs).
LIFE (Living Independently for Elders) is a Program of All-inclusive Care for the Elderly (PACE), which assists older adults in living safely at home. Eligible participants are 55-years old or older, live in the designated service area, meet the state requirements for nursing home care and are able to live safely in the community. LIFE is designed around the needs of the individual and provides comprehensive medical, health and social services (including adult day health programs, rehabilitation therapy, nutritious meals and social engagement) to participants at the center as well as in their homes LIFE services are paid for by Medicare, Medicaid or private payment. There are little or no out-of-pocket charges if participants qualify for both Medicare and Medicaid or Medicaid only. For participants who do not qualify for Medicaid, there is a flat monthly fee.
MINIMUM QUALIFICATIONS
1. Medical license. CDS Certification, DEA Registration and current BLS required
2. Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.
3. A minimum of one (1) year working with the frail or elderly population required. 4. Previous supervisory experience required.
