Docstar is working with a client of ours that is looking for a Physician to work in New Hampshire, and specifically focus's on Geriatric population.
- 3 half days a week, doing inpt geriatric consultations with focus on decreasing length of stay by identification and management of geriatric syndromes, polypharmacy, and goals of care. Coordinating consultations with dedicated RN and another geriatrician. Taking part in interdisciplinary rounds
For SNF/LTC – daily rounding in SNF/LTC; doing SNF admissions, at least weekly follow up (optimally twice weekly) on SNF pts, SNF discharges, acute visits, long term care routine follow ups. Coordinate with current SNF MD. Nighttime call – one week at a time, phone only (doesn’t need to go in). Right now the focus is on a facility 2 miles from the medical center. Potential for some work in a second facility 4 miles from the medical center. Familiarity with Epic optimal.
- M-F, 40 hrs per week. 3 of the days are inpatient consults, the remainder of the days/week are clinic
