Our Purpose in St. Louis
Bloom Healthcare is expanding into the St. Louis area to bring high-touch primary care to patients in senior living communities, helping medically complex seniors age in place. We are hiring a Physician to join our growing clinical team and deliver relationship-based care where it matters most—at home and in community settings.

Bloom offers a different path for patients and physicians alike: longitudinal care relationships, more time face-to-face, and more dignity at home. We provide this under a population health model, which allows doctors to get off the “hamster-wheel” and reclaim balance in their practice. This is an opportunity for a physician who wants to practice excellent medicine and join Bloom as we thoughtfully expand in St. Louis—bringing an established care model that’s been refined over time (with roots in physician house calls dating back to 2003).

Job Responsibilities

The Opportunity
As a Physician, you’ll provide direct patient care across home and community settings while partnering closely with nurse practitioners and operational support to deliver coordinated, high-quality care. You’ll have the autonomy to practice thoughtfully, backed by a team and infrastructure designed to keep your focus on patients—not logistics.
How You’ll Make an Impact

Clinical care & quality
-Deliver primary care to seniors in their homes and community settings (Assisted/Independent Living)
-Provide evidence-based, coordinated care with a strong patient experience
-Support clinical consistency through documentation standards and practical workflows
-Collaborate with an interdisciplinary team to support care coordination, medication management, and smooth transitions of care

Team collaboration
-Partner effectively with nurse practitioners and operational teams to provide seamless, patient-centered care
-Contribute to a positive, collaborative team environment through clear communication and a steady clinical presence
-Support continuity of care by sharing clinical insight and maintaining consistent standards across the care team

Qualifications

-MD/DO with board certification in Internal Medicine, Family Medicine, Geriatrics, Hospice, Palliative, or related primary care specialty
-Eligible for licensure in Missouri and able to practice in the St. Louis region
-Comfort working with medically complex older adults (multi-morbidity, polypharmacy, care coordination)
-A patient-centered clinical style: thoughtful, reliable, and team-oriented
-Bonus (not required): home-based primary care, geriatrics, value-based care exposure, or experience working alongside APPs
