AbsoluteCARE is seeking a dynamic, mission-driven Infectious Disease physician to put the principles of team-based care and population health into action for our medically complex & socially vulnerable members. Unfettered from the traditional fee-for-service model, through unique provider-payor relationships, AbsoluteCARE is designed to care for society’s sickest patients. Prior to coming to AbsoluteCARE, our patients are typically ill-served by the uncoordinated traditional health care system, bouncing between emergency rooms, hospitals, and specialists several times a year. But through our integrated care teams, including physicians, nurse care managers, social workers, behavioral health specialists, pharmacists and community outreach workers, we are able to transform the lives of our members. We offer complex, coordinated medical care through our medical teams with our onsite infusion center, full pharmacy, and lab & imaging suites. We also treat the social determinants of health, addressing housing instability, food insecurity, or simply loneliness through our integrated care team.


Responsibilities:

  • Providing in-person and telehealth infectious disease consultation for primary care teams across offices, including for HIV, Hep C, complex antibiotic therapy.
  • Responsible for the primary and ID care of your own panel of members, with the assistance of your primary integrated care team currently consisting of 2 case managers (RN's), social worker and a medical assistant.
  • Oversee infusion services, including for IVIG and IV antibiotic therapy
  • Focuses on the management and improvement of health for patients with multiple complex and chronic medical conditions.
  • Able to work in a team environment (with social work, case management, behavioral health, pharmacy, nutrition, etc.), process feedback, and be open to discussing and implementing plans of care from other members of the team.

Qualifications:

  • Licensed physician in the state of Maryland
  • Board certified Internal Medicine and completion of an Infectious Disease fellowship
  • AAHIVS Certification preferred
  • BLS certification required
  • Strong focus on customer service is required
  • Experience in population health management is preferred
  • Experience in working with psycho-socially and medically complex patients is strongly preferred
  • Knowledge or experience with NCQA PCMH model is preferred
  • Must be able to work as part of a multidisciplinary team with constant collaboration within and across provider teams
  • Experience and comfort around maximizing EMR systems
  • Problem-solving and the ability to be creative in those solutions
  • Strong oral and written communication skills
