Program Overview The Horizon Program provides a full spectrum of medical and social services for HIV-positive patients across central and mid-coast Maine. The program is designed as a “one-stop” care model, integrating multiple services into a single, coordinated patient experience. Services include: • Primary medical care • Mental health services • Case management • HIV/AIDS treatment and long-term management • Care coordination and patient support services This integrated approach improves access, outcomes, and patient experience by addressing both medical and social determinants of health.
