Advanced Practice - Hospitalist position in: Hartford, CT

Clinical Responsibilities • Provide full-spectrum inpatient hospitalist care. • Triage and admit patients. • Perform daily inpatient rounding. • Manage patient discharges and transitions of care. • Evaluate and address changes in patients’ clinical status. • Perform physical examinations and develop appropriate treatment plans. • Develop medical, educational, and psychosocial plans in collaboration with physicians and the care team. • Coordinate further evaluation and referrals to specialists or other healthcare professionals when appropriate. • Provide treatment within established protocols and scope of practice. • Identify conditions outside the provider’s scope and appropriately refer to a physician.
