**Post-Acute Hospitalist Opening in PA**
- Located in Reading, PA; Philadelphia 60m
- BE/BC
- Full-time; Permanent position
- Block schedule of 10-hour day shifts
**Overview**
- Works with a team of 6 Hospitalists and 11 Physiatrists
- Admit & manage clinical care of Adult Medicine Service patients on a sub-acute unit, with average LOS of 13 days
- Comprehensive medical consults in a co-management model on acute rehab patients with Physiatry
- Provide care to patients on the Traumatic Brain Injury Unit
- Discharge planning, including ensuring appropriate follow-up
