Locum Tenens Acute Care Advanced Practice Provider (NP/PA) – Remote
Locum Tenens Acute Care Advanced Practice Provider (Nurse Practitioner or Physician Assistant) – remote telemedicine role supporting a Hospital at Home program in California. Ideal for experienced hospitalist NPs and PAs with acute care experience who want flexible locum tenens work and to make an immediate clinical impact.
Job Details:
- Location: Fully Remote; Must live in CA and be available to travel to Walnut Creek, CA for 2 days of in-person training.
- Employment Type: Locum Tenens (contract position - ongoing)
- Setting: Remote/virtual patient intake and telemedicine (Hospital at Home program) with 2 days of in-person training in Walnut Creek
- Schedule: 7 days on, 7 days off
- Shift Duration: 10-hour days (7:00 AM – 5:00 PM PST)
- Start Date: May 25, 2026
- Duration: 53 weeks (with potential for extension)
- Compensation: $115-$125/hr W2
Qualifications:
- Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
- Current NCCPA certification for PAs or Acute Care NP certification for NPs
- Active California state licensure as a Nurse Practitioner or Physician Assistant (CA license required)
- Valid DEA license
- Must reside in California and be able to travel to Walnut Creek for initial in-person training
- Minimum 2 years of recent hospitalist acute care experience
- Telemedicine / telehealth experience and remote patient assessment experience highly desirable
- Proficiency with electronic health records (EHR), especially EPIC
- Strong clinical assessment, diagnostic, documentation, and communication skills; experience with care coordination and remote patient monitoring
Key Responsibilities:
- Perform comprehensive virtual assessments to evaluate patient medical status, history, and presenting symptoms
- Review medical records, lab results, imaging, and prior notes in the EHR to inform clinical decisions
- Determine eligibility for the Hospital at Home program using established clinical criteria and triage protocols
- Develop individualized care plans and coordinate enrollment in home-based hospital services
- Provide remote monitoring, virtual follow-up assessments, and ongoing management in collaboration with multidisciplinary hospitalist and home health teams
- Communicate care plans, risks, and next steps clearly to patients, caregivers, and family members
- Coordinate with primary care physicians, specialists, and community providers to ensure continuity of care
- Document all patient interactions, assessments, and care plans accurately in the EHR to meet regulatory and quality standards
