We are seeking a physician leader to join our team as associate medical director in Albany, New York. We ask that you have the ability to work in-person within a 30-minute commute of Albany 5 days per week. Competitive compensation with an estimated base salary range of $179,000 to $318,186.

Position Overview
- Full-time (5 days/week) rounding in skilled nursing facilities

Key Responsibilities
- Perform patient assessments, histories, and physical exams, develop and adjust individualized care plans, monitor patient's progress, and coordinate care for optimal outcomes
- Educate patients and families on health management
- Maintain accurate, timely medical records

Qualifications
- Current NY physician medical license and DEA
- Experience in post-acute, acute, EM, or clinic settings preferred; new grads welcome
- Strong clinical skills and communication abilities
- Ability to commute/relocate: Albany, New York; relocate before starting to work (required)
- Work location: in person, full-time, W2 eligible only
