Location: New Haven Area, Connecticut
Schedule: Full-Time | Permanent

Organization Overview

This position is part of a respected, community-based healthcare system serving the Greater New Haven area. The organization is well regarded for its compassionate, patient-centered approach and maintains strong clinical ties with the region’s leading academic medical community, including access to resources associated with Yale University.

The hospice and palliative care program offers comprehensive symptom management, goals-of-care discussions, advanced care planning, and end-of-life care support. The program works closely with inpatient hospitalist teams, case management, oncology, critical care, and community hospice agencies to deliver coordinated, evidence-based care.

Position Summary

The organization is seeking an experienced Hospice & Palliative Care Physician who can also provide Hospitalist coverage to support its growing inpatient and palliative services in the New Haven area.

This full-time, permanent role involves caring for hospitalized patients requiring palliative consults as well as covering general inpatient medicine shifts. You will join a dedicated team of clinicians who value collaboration, empathy, and high clinical standards within a supportive, multidisciplinary environment.

Essential Responsibilities

  • Provide inpatient palliative care consultations, including symptom management and goals-of-care discussions

  • Participate in family meetings and coordinate care with interdisciplinary teams

  • Deliver evidence-based hospitalist care during assigned inpatient shifts

  • Support transitions to hospice and collaborate with hospice agencies

  • Communicate effectively with nursing, case management, social work, and specialty teams

  • Maintain accurate and thorough EMR documentation

  • Participate in quality initiatives, clinical reviews, and care improvement efforts

  • Contribute to a multidisciplinary environment aligned with high academic standards

Qualifications

  • Education: MD or DO from an accredited medical school

  • Training: Residency in Internal Medicine or Family Medicine

  • Certification: Board Certification in Internal Medicine or Family Medicine required; Hospice & Palliative Medicine fellowship or substantial experience preferred

  • Licensure: Eligible for Connecticut medical licensure

  • Experience: Hospitalist or inpatient care experience strongly preferred (facility not considering new graduates )

  • Skills: Excellent communication, empathy, and team-based care approach
